Our purpose is to assist individuals in returning to health and quality of life by helping them understand their condition, to discover what is best for them individually, and to carry out a treatment plan to achieve their goals.

Office Hours: Monday thru Friday 8:00 am -5:00 pm. We have late afternoon
appointments for our working families and school children.

Insurance Companies: Dr. Cook is currently contracted with most major PPO
insurance companies. Please call our office for more information or contact your insurance carrier for your contract information.

Surgery: Dr. Cook performs outpatient surgery at the Orthopedic Surgery
Center of Orange County in Newport Beach as well as at Hoag Orthopedic
Institute in Irvine and Hoag Hospital in Newport Beach.

Worker's Compensation: Dr. Cook treats work injured patients. He is a state
QME and provides IME services.
